Notice and Permission form for Dance Team Performances and Competitions

Dear parents and guardians,


The dance teams and I will be visiting Victoria May 5 – 7th. In order to move forward with the planning for the Victoria field trip, students need to complete two tasks by January 12, 2017:

  1. Have parents sign the permission form. (I will keep the form until semester 2 and then have you get your teachers to sign your permission form) 
  2. Have parents check the box on the permission form which indicates that you are allowed to attend the Victoria Team Trip. By giving your child permission to attend the team trip you are also confirming that you will be paying up to $150 for the trip. (Or your child will be doing lots of chores for you to earn the money.)

Upon submission of this registration form, it is assumed that all team athletes have permission from their parents or legal guardians to participate. It is also understood that Surrey Secondary Dance Teachers’ Association, coaches, staff, and volunteers shall not be responsible for any injury incurred as a result of the athlete’s participation in the event, nor in the event of loss, theft or damage of personal property. All dance teams should have their own general liability and accident insurance for their members. Photography and videography will also be taken at the events.

Field Trip: November 24: The Dance Centre

Itinerary

8:30am Meet in the Dance Studio
8:45 - 9:45 Bus to The Dance Centre 
10:00 - 11:00 Workshop with Lesley Telford
11:00 - 11:45 Lunch - meet back at The Dance Centre down stairs in the lobby
12:00 - 1:00 Watch Performance: The Response 
1:15 - 2:15 School Bus back to Frank Hurt 

Bring: 

  • Dance wear
  • outdoor clothing 
  • water
  • food or money for food

More Information: 

Workshop with Lesley Telford  Improvisation / creation
Time:                      10:00 – 11:00am  (The studio will be available 15 min prior and after the class)

Location:               Birmingham Studio, Level 3 (large studio at the end of the hall)  

Change rooms:    Women’s (Level 3), Men’s (Level 4) When you exit the elevator on both levels, the change room is to your left hand side.

Studio etiquette:   No food or drink allowed in the studio, with the exception of a water bottle
No marking shoes are permitted on the Marley floor;  Barefeet or socks only
Dancers will be able to bring their personal items into the studio and can line them up below the window.

Could you make sure your students are gathered together in the Faris Lobby by 11:30am so that we can sit your group all together if possible.
